and mail it along with your check to: The Well Project, P.O. Box 768, Nellysford, VA 22958.
The charitable contribution form is a PDF document and requires Adobe Reader to view and print.
Adobe Reader is a free software program. If you do not already have Adobe Reader software on your computer,
download Adobe Reader now.
You may also request that a copy of the charitable contribution form be sent in the mail by contacting us.
